You're right, the env var isn't the deciding factor. Without
`KWIN_USE_OVERLAYS=0` the flashes still happen; my earlier clean period was the
re-login or chance.

It also doesn't look like KWin. With KWin 6.8.1, Mesa 26.2.5 and firmware
unchanged:

- 7.3.0-4-cachyos: flashes reproduce repeatedly when windows open, close or
take focus (e.g. a loop opening/closing a Konsole window every 3 s).
- 6.18.55-2-cachyos-lts: no flashes in 80+ minutes of the same kind of use.
- Back on 7.3.0: flashes again.

Two KWin screencasts made while I saw 3 flashes show no dark frame at those
moments, so KWin's composited output looks correct. drm.debug captures (0x14
and 0x102) show only routine page flips at the flash times. I've reported it to
drm/amd as a likely 7.3 display regression:
https://gitlab.freedesktop.org/drm/amd/-/issues/6044

I think this can be closed as not a KWin bug. I'll reopen if it points back to
KWin.
