After selecting the option earlier, I couldn't force the bug by switching to a TTY and back, but after I rebooted to test KWIN_USE_BUFFER_AGE=0 in my /etc/environment, and switching back and forth between "Automatic," "Only when cheap," and "Full screen repaints," I saw the issue on all three - which also means that KWIN_USE_BUFFER_AGE=0 doesn't help. kwin_x11 --replace still works to fix it.
