I'm also running into the same issue. It seems that the first window opened
after a suspend is the one that has the title bar missing. All further windows
opened are fine. It doesn't seem to matter what program is run, just the first
one window after a suspend. I'll attach an example too, called "chrome.jpeg".
I have a keyboard shortcut set to run "kwin_x11 --replace" and this fixes the
black title bar in the window.
