I suspect that there may be a timing issue involved too. I use a USB external sound card with phonon and the gstreamer backend - on this laptop the VLC backend doesn't work well. Most of the time I have no problem, but from time to time I get those messages that something doesn't work and it's falling back to default settings. Two good things to remember - Phonon handles simultaneous sound inputs, so for many things if it fails you will be absolutely fine with the ALSA that it falls back to. Secondly, the Phonon maintainer's blog implies that he got a lot of improvement at the recent sprint, and we will see improvements in the next release.
