That's actually an interesting question. It's a bit hard to do that, though, because there are no obvious search terms. Searching for "refrain" turns up 6 commits in the 4 years of kde-commits history I have here (a bit more than 360000 commits), where a maintainer complained about code changes done by others without review (usually in the form of "please refrain from doing ..."). That's not particular frequent. So even when assuming that there are 50 more such queries giving different results, this is still in the order of magnitude of every thousandth commit.
