No, there is no Qt4 flag. The output of the configure script should tell you if 
MLT is compiled with Qt4 support. It should look like that:

Configuring modules/melt:
Configuring modules/motion_est:

Configuring modules/oldfilm:
Configuring modules/plus:
Configuring modules/qimage:
Qt version 4.x detected, will compile Qt4 qimage producer
(...)

Then the Qt4 module will be compiled and installed. Are you sure you don't 
have several installs of MLT (one in /usr, and one in /usr/local for 
example...).

MLT might be looking for the modules at the wrong place...
