First of all, it is a major issue, but not the problem of KMail but Reiserfs.
Anyway, it affects users and affects badly. Here is the original report:

Anyway, the problem is with any program that creates a lot of small files -- I
use only two ccache and KMail with maildir type of mail folder. In theory it
should not happen, but in practice it does and more time passes (more mail I
have) I have more and more problems with saving anything on disk.
This could also be true for ext2 and other similar (to ext2) filesystem.
