When adding an attachment to an email the file is attached as soon as it is
selected with a SINGLE click in the Dolphin window.
I would suggest that the correct operation should be to select the file and
then press the 'attach' button.
Or DOUBLE click the file to attach it.
The way it currently operates increases the chance of the wrong file being
attached.
I have seen similar behaviour in other KDE applications, such as when adding a
custom cover in Amarok.
