I opened akonadiconsole browser and somehow found I had both a trash and
wastebin folder in my local folders (despite using only imap and not the local
folders). After deleting the trash folder, restarting the akonadi server and
killing off a stuck kmail instance, all the hanging and high cpu usage mysqld
process went away. Seems in my case the problem was due to there being multiple
wastebins under different names.
