Brad, I do try to stay up to date. These kinds of bugs are really rare for me but I guess it still worth reporting them. When it happened after your last comment & my last upgrade, apport (?) did not recognize an existing bug but created another one https://bugs.launchpad.net/ubuntu/+source/linux/+bug/1127496 . I'm not sure if these are related including https://bugs.launchpad.net/ubuntu/+source/linux/+bug/1103492 .
