iwlwifi: add new -21.ucode for 7265D, 8260, 3168 and 8265 devices
This firmware is the first firmware we release of 3168 and
8265. It is supported starting from kernel 4.6.

Currently we have iwlwifi-8000C-13.ucode and iwlwifi-8000C-16.ucode in
Xenial, and iwlwifi ask for -19 for certain cards.
