I did send this in as private but if its been changed to public as long
as i can get an answer to what might be causing the repeated issue I
would be greatful either way at this point ...

Bug description:
  its been crashing system for awhile each time 
  first i thought it was a bad superblock but it won't let me fix it
  says-in the initramfs to run fsck manually-  when i try to do the fsck code 
error i got
  was /dev/mapper/ubuntu --vg-root contains a file system with errors check 
forced 
  inodes have a part of a corrupted orphan linked list found 
  the fsck fails exited with status code 4 /dev/mapper/ubuntu --vg-root

  so i start the sudo fsck code but it says there is no sudo and locks
  me out of everthing

  changed partitioning i had to something completely different  and then it 
crashed again yesterday
  after i logged out two other times it was while using gimp or my paint
  and i spent hours over normal it wouldnt delete 2GiB of data on the wiping 
  of drive through PartedMagic 
  but I finally thought last night i got ubuntu back up yet today
  it says there was a error in it when i created the OS back
  hope you can help with this bug report 
  i am at a lost to why?
  not sure if its security issue or not but reporting that it is
  because i didn't think that should happen
  i have checked the actual hdd and it says its passed and fine in PartedMagic 
  if its to crash again 
  reporting this cause i just don't know what else to do
