We're seeing this on our build servers. These are VMWare virtual machines which run docker containers in privileged mode. Inside the containers are Jenkins agents and a build environment. Part of the build process uses unshare to create these namespaces: ipc uts mount pid net. If you have any questions about this arrangement, I can try to provide more detail and maybe example code.
The kernel is 4.4.0-79-generic x86_64. We are using docker 17.05.0-ce. In one case, the whole VM hung when this was encountered.
