I'd estimate that this happens perhaps one in every two or three resumes, so it's pretty annoying. I'm currently running 18.04, but was on 16.04 up to a few months ago. I suspend relatively infrequently so I don't have great data, but my sense is that this happened occasionally on 16.04 but has become very much more frequent on 18.04. I haven't been able to find any useful log information about what the problem might be: syslog has log entries for suspending and then goes straight into the log of the next successful boot, with nothing that I can see relevant to the failed resume. I'm willing to dig given specific pointers, though.
