Title:
  USB 3.0 flash drive not mounted

Status in linux package in Ubuntu:
  Confirmed

Bug description:
  When I insert USB flash drive to the USB port nothing happens. The
  drive is not listed by `lsblk`.

  dmesg reports:
  ```
  [ 1262.090351] xhci_hcd 0000:04:00.3: Error while assigning device slot ID
  [ 1262.090371] xhci_hcd 0000:04:00.3: Max number of devices this xHCI host
supports is 64.
  [ 1262.090378] usb usb3-port2: couldn't allocate usb_device
  [ 1280.075712] xhci_hcd 0000:04:00.3: Command completion event does not match
command
  ```
  Ubuntu 18.04 LTS
  kernel: 4.15.0-29-generic

  Interestingly when I reboot while keeping drive in the port it is
  mounted after reboot and fully operational.
