Bug description:
  We have had this problem happen 3 times in the last 5 months. Each time it 
happens, soon after, processes start getting stuck in IOWAIT and eventually we 
have to reboot the machine to get things working again. The system was running 
for years without problems as a VMWare guest running Ubuntu 14.04. In 9/2018 it 
was upgraded to Ubuntu 16.04 with a 4.4 kernel. Then in 4/2019 we installed 
linux-generic-hwe-16.04 to get the 4.15 kernel and it was migrated to a hyper-v 
server. The first crash was logged on July 29 2019, the second on Sept 17 2019, 
and the most recent on Nov 1 2019.
  ubuntu-bug linux reports "Problem in linux-image-4.15.0-67-generic This is 
not an official Ubuntu package." but it is the kernel package apt received from 
Ubuntu's servers.
