You have been subscribed to a public bug:

External monitor connected with HDMI goes black when cable is
disconnected and reconnected.

Same thing happens when the screen saver kicks in.

Gnome still thinks that there is an external monitor and an extended
desktop, but the external monitor is black.

Work-around:

Disconnect the HDMI cable, reboot and then connect HDMI.

This is probably a duplicate of bug #1992563.
