buf04.patch - BUF/BIO patch #4 (commit candidate)

    Here is the fourth iteration of the BUF/BIO patch.

        fetch http://apollo.backplane.com/DFlyMisc/buf04.patch

    This fixes all known bugs to date.  This is now a commit candidate,
    so please test!  The patch will be tentatively committed Thursday
    evening.

    Thanks to Stefan Krueger for doing major testing of UFS, MFS, NFS, and
    CAM.
