Hi Garth,

The wxFormBuilder version I'm using is 3.5.0-beta. I don't have any
issues on Ubuntu (obviously) and I've just installed the same
3.5.0-beta on Windows and the dialog appears fine.

The only OS I cannot test on is Mac. What OS are you using?

Perhaps another developer can also take a look at the dialog to see if
they're experiencing the same problem.
