thanks for the suggestions.  Having messed around installing both
PDFCreator and FreePDF XP, I realised that PrimoPDF was in fact
available, it was just that dialog with the list of printers was 2
columns and 3 rows, but with only 1 column displayed.  Apologies.

Still, I can make the following conclusion which may be useful :)
 1. FreePDF XP has the fewest settings, and is too basic for me, and
the installation is more complicated
 2. PrimoPDF has a nice user interface and a sensible number of settings
 3. PDFCreator has lots of settings, which makes it a bit harder to use
