I had a similar problem this week when trying to add a capacitor to a 
schematic. Although the symbol in the schematic showed an annotation of C22, 
running ERC resulted in an error that annotation was needed for that symbol. 
When I opened the Component Properties dialog for it, the reference field had a 
value of C?. I tried probably 3 or 4 times to set it to C22, and each time it 
failed to take. Finally, I manually edited the schematic file and afterward had 
no problems. So it appears that some problem exists in regard to schematic 
designators.
