After upgrade to 3.16, We have a number of reports of Libraries who are seeing
fines forgiven with accounttype 'FFOR', i.e. 'Fine FORgiven'. The libraries
reported that they were not checking the 'Forgive Fines' button on the circ
screen; we have hidden that check-box, and we are still seeing additional
records in accountlines with accounttype 'FFOR'.
