I think this problem only happens after you've changed the width of the browser and buttons in the toolbar have wrapped to a new line. I think this is a relatively uncommon sequence of events, and a page refresh fixes it. This problem needs to be fixed globally for every page which has a floating toolbar (see also Bug 21751).
