Questions:
- Do we need a fulfillment step at checkin? i.e. if an item that can fulfill
the request is scanned, then it is informed to the staff, and assigned to  the
AR.
- In that case, wouldn't it make more sense to charge the fee when the AR is
'accepted'? Because if you have a default fee for the biblio-level itype, and
then the staff assigns a specific item, then the fee may differ?
- I understand not all AR need to be fulfilled by specific items, because the
library might have already scanned things. In that case, how it should behave?
Maybe for another bug.
