Thanks Martin! Good call - I think this is pretty minor in the scheme of
things!

I've signed off, but the DBIC patch no longer applies - I skipped this and then
ran dbic (things are moving so quickly!)...

On the OPAC for the report a concern form, the template text is now below the
input form and help text - I'm assuming it is to do with the DBIC patch not
working. If not, I'll let you add a follow-up patch!

I'll also attach an updated consolidated testing plan/notes, if this is any
help for the QA Team and documenting.
