When items are created on receiving serials, the item form is misaligned. I
think we fixed very similar issues with the item form in cataloguing and
acquisitions, so maybe just missed this form.
To test:
* Create a subscription with 'add items on receive'
* Receive an item
* Verify item form is misaligned
