I think Pedro's suggestion to clarify the MARC modification templates functionality is nice - but I also think it belongs in a separate bug. As Esther's thorough testing highlighted (in comment #34), there is already a way to make sure only one item is created: you simply need to use "Update existing or add new field" in subsequent actions rather than "Add".
