To recreate:
- find a patron record and click Suggestions in the left navbar
(/cgi-bin/koha/members/purchase-suggestions.pl?borrowernumber=XXX)
- click "New purchase suggestion"
- link points to
/cgi-bin/koha/suggestion/suggestion.pl?op=add&suggestedby=XXX&redirect=purchase_suggestions&borrowernumber=XXX
- page does not display a new purchase suggestion form
