Passing QA here. Test plan: 1. Navigate to Tools > Patron clubs 2. Create a club template 3. Create a new club using new club template 4. Observe the date fields are "start date" and "end date" 5. Apply patch 6. Refresh page, observe that the date fields have changed to "Enrollment start date" and "Enrollment end date".
