Locally, we have a opac-memberentrytype2.pl script, which uses a duplicated set of system preferences, but it doesn't work so well when you want to use different extended patron attributes. Plus it would be nice to make N forms instead of just 2. Another thing to keep in mind is that it's not just self-registration. We'd also need multiple self-modification forms. Of course, we could look at first migrating the system preferences into a different display, and then worry about extended patron attributes. Hmm... need to put a bit of thought into the design and how this would all work. But yes... very interested in this idea.
