Vitor, I apologize if you already do this, but I highly recommend you use a version control system to monitor any changes you make to a package's source files. And of course for any add-ons, scripts, plug-ins you program yourself.
I even do this for managing my configuration files (http.conf, vhosts, php.ini my.ini etc. I personally use Subversion, but Git's one of the latest hotness if you're starting from scratch, another one's Bazaar.
