I think the place to start for this enhancement would be to look at how it's implemented in the Suggestions management area of Acquisitions. You can select the reason for accepting/rejecting a suggestion from a dropdown connected to Authorised Values, but you can also input your own text. The Messages section of Borrowers also has this capability.
