The import csv must contain the attributes you wish to load in the patron record from the columns in the manual: http://manual.koha-community.org/3.16/en/tools.html#patronimport
Column categorycode must match the categorycode corresponding to the patron attribute in http://manual.koha-community.org/3.16/en/patscirc.html#patcats. You must also match the branchcode if you have more than one library. Special attributes must match a corresponding column titled patron_attributes. In that column the attribute is listed with the CODE followed by a colon and then the value. Each attribute is separated by a comma. Example: patron_attributes ID:22222,GRAD:2015,CLASS:SENIOR Hope this helps.
