I just installed Debian-live-8.7.1-amd64-kde-desktop.iso on a virtual
machine and Koha 16.11.03 from the Debian packages, and mysql 5.5. I
reloaded my koha.sql data from the old system 3.22.05.
The upgrade went with a few errors:

AuthorisedValueImages or StaffAuthorisedValueImages could be turned off

I followed the wiki and Kyle's additions to set up plugins.
I tried to install my DonorApp plugin. It put the plugin in the proper
place but no plugins showed up on the Tools Plugins page. I tried
ByWater's kitchen-sink-v1.kpz with the same result.
