I have enabled the file mpm-itk and when looking is the apache sub-directory I found a README.Debian file that stated this was no longer a true MPM file, but was enabled as a prefork.
Note that mpm-itk has gone from being a true MPM to just being a module that hooks into prefork. You can safely remove the old apache2-mpm-itk package if you still have it installed. So it appears that there is no file mpm-itk to enable.
