According to an earlier message in this mailinglist (see https://lists.katipo.co.nz/public/koha/2017-August/048715.html - also see http://koha.1045719.n5.nabble.com/Patron-Add-error-tt6001596.html) this error appears when you are submitting a form without coming from the form. That should not happen.
