After you empty or delete the duplicate records , do you try to
continue the upgrade ?
Either from web installer or cli ?

I went through the language tables and removed any apparent
duplications. I also emptied out the language_descriptions table
with no success. Thanks Jonathan for opening the bug ticket.
Fortunately my system is for internal institutional use and I can be
patient.

I had similar problem and I found that on the table
"language_descriptions" was the problem , because on bug 15607 is
trying to generate unique indexes and if you have a double value
there you have a problem like that you described. You should remove
the duplicate value and re-run your upgrade though web installer.
Also I suggest you to check for double values other language_* tables.
