Yes, we are always using Plack.

Yes it happens all the time - at least if someone uses the OPAC and/or
the staff client. But until now it is not possible to find out what
exact page triggers that message - sometimes I call a page and the
message appears, sometimes not. Sometimes five or more of these messages
appear at once. There are usually only a few seconds between one or more
of these messages. And the logfile contains thousands of them.
