Are you using pop-before-smtp? The reason I ask is that Oulook does
things backwards in typical M$ fashion. It trys to send before pop,
and if you are using pop-before-smtp it will fail on first try
sending, since the machine isn't yet authenticated with the pop
server.

That's typical, and likely unrelated to Postfix.
