Last time I checked, it worked best if you ran the livecd generation tools on Fedora 7. It's likely that the latest version of koan in git supports Fedora 8, which had some incompatibilities. I personally had to patch it to get it going and it wasn't difficult. I don't think the requisite Fedora tools are present on EL5 or other distros for it to work, so your best bet is to throw F8 somewhere and start from there.
