I'm with Cox Cable/Internet/Phone in Santee. Speeds were damn near intolerable before I replaced my 8-year-old Toshiba cable modem with a new D-Link last year, and that simple $40 swap-out brought me from ~5mbit to nearly ~20mbit for downloads, and from ~200kbit to ~550kbit uploads.

Overall I'm pretty happy (especially when downloading ISOs), but Cox is my only option for broadband where I live, and I use their internet services as minimally as possible (i.e., none of my email goes through their systems, ever.)
