The idea is not bad. I don't know what kind of reception it will get.
One change I would recommend: square up the check boxes for Topics of
Interest

Current:
 _ install  _ config  _ shell (command line)  _ finding apps  _ using apps  _ 
security
 _ web development    _ programming  _ other:

Recommend:
 _ install  _ finding apps  _ programming  _ web development
 _ config   _ using apps    _ security     _ shell (command line)
 _ other:

It does use one more line, but it looks a lot cleaner and neater. It also
gives more room for other. Make sure to offer enough whitespace for
someone to fill in other by hand.

The one thing I am not certain about is lines for general comments and
suggestions block. If you do offer lines, make them 50%-75% gray.
