 I agree.  I have seen those fails also.

If you put two or more functions on one switch,  be  prepared to loose all
the functions upon failure of the switch.  You might also loose functions
on the buss connected to that switch when the switch grounds out or opens
one or both functions or shorts between functions.  Use circuit breaker
placement planning to limit the scope of failed circuits.
