-drive cache=none results in QEMU opening the image file with O_DIRECT. It's not clear to me what the first error is. The node-installer output you have posted does not show a series of steps to reproduce the problem. Can you try running isolated tests with dd or verify-data (http://people.redhat.com/sct/src/verify-data/)? Have you tried running a different guest OS, say a Fedora or Debian Live CD?
