I had a problem that sounds exactly like what you're talking about. In my my case it wasn't as benign as simply going off-hook randomly. In my situation the extension would actually pick up incoming calls (or intercom pages). Of course, that meant that the incoming call would be connected to a phone that really hadn't been picked up. When the caller hung up, the 624 would drop the line and the extension would switch from the outgoing call to intercom and remain off-hook. This happened with all of my extensions, but some seemed more susceptible than others.
Unfortunately, I never did get to the bottom of what was going on. I tested all the wiring for shorts, I swapped out extensions, nothing helped. I eventually swapped out the 624 and the problem went away. It was suggested that the cause might have been related to a non-standard voltage/current on the CO lines, and that my original 624 was a bit hyper-sensitive to it. That didn't make a lot of sense to me since the problem occurred with pages and the doorbell as well, but you might want to try a filter on your CO line(s). (I never did this, so I don't know if it would help). I suppose (as someone else suggested) it could also be a problem with the EEPROM version... My new box has a -4 in it while the "bad" one had the original version. It could also be as simple as a variable resistor or capacitor set wrong somewhere, but I wouldn't know where to look. I would have loved to have been able to talk with Panasonic tech support about this (they probably know exactly what's going on since it doesn't seem like an isolated problem), but I'm not a dealer so I didn't have access. (I inherited the system so it had no warranty, and the original dealer gave me the impression that attempting to fix it would likely be more trouble than it was worth.) I'd love to hear if you get to the bottom of this, as I still have the old system sitting in a corner somewhere.
