Make sure the amphenol plug is in all the way (top and bottom).  They do not
attach very well and can come loose if the cables are bumped, then some or
all of the extensions on the card may go dead. I usually try to stuff some
cut foam plastic (from a packing box) in to hold it in place when I put the
cover back on.
