Sorry for the delay, just deleting old msgs and
spotted this one.  I might have a answer.

There is an opto coupler in the CO circuit that
"connects" the line when you press a co button.  Over
the years, we had two of these fail "open".  

The symptom is that an incoming call will ring as
normal, but when you answer the call, the light lites
etc, but they can't hear you.  Actually, when you
"answer" you will hear a loud "brrup" for 1/10 second
before you hear them saying "hello - hello - hello". 
Caller can't hear you.  When trying to make an
outgoing call, you can hear some central office line
noise, but never pull dialtone. (you have an audio
path but no DC path)  

If you are so inclined, it's a 4 pin device marked
with 627 and can be unsoldered and replaced. There is
one for each line, four to a kxt123280 card or kxt180
module- hook your toner to bad co, then find the one
singing ;-) Pano parts has them listed, but you can
get them from Radio Shack national parts as a TLP627.
I think Panos price was about $24, RS was like $6 for
1 or 2, of course it's an obsolete $0.50 part. RS used
them in one of their phone devices.  If it helps, the
ones we replaced were white moulded plastic, haven't
seen a black one that failed.

You can test them by shorting the two pins on the side
AWAY from the dot while a phone is picking up the bad
line, the line will work while shorted.  (Actually you
can short either side without harm, just don't short
anything end to end! -and be careful not to short all
the other stuff around it )
