It can be related to info_zombie_spawn entity. If you spawn too much common infected you will surpass engine limits. The maximum horde number is 50 in finales and 30 if it is a common horde, like from Boomers. Always use vscripts to manage hordes and gauntlets to avoid it.
I know it because original L4D1 Heaven Can Wait campaign has this problem. In L4D2 you can use vscripts to simulate gauntlets (unlimited hordes) but in L4D1 not.
