It does; I've determined that repacking the model files straight from L4D2 into the VPK fixes the problem entirely as suggested. Is this possibly an issue with the way VPKs are currently reading? I do feel a bit silly including most of the melee weapon view models in my map that should be accessible right from the game, in particular when it's due to such an unusual bug. I can't imagine why map position would impact which weapons work and which do not.
Anyway, I do have a work around now, but this is really a fascinating bug to me.
