The fact the puppy was only 5 weeks old when you got it may be part of the 
problem.  Those last 3-4 weeks with mom and sibling are critical for puppy 
socillzation and pecking order establishment.  Seems like she had no respect 
for you and feels he/she is higher than you in the pecking order...something 
you will need to learn how to change.
   
  Your trainer should be able to help with that.........Lot of books on the 
subject.
   
  You must catch her in the act.  Time out is a child's punishment not a 
dogs.....the crate must not be used for discipline at all in my 
book.......Praise good behavior and disipline/correct or redirect bad 
bahaviors.....

better but from bitting to this is no longer something we can continue to
